car starts ONCE after flash

Tracing a big pain in my a**! Using EEHack I flash a .bin file into my pcm and the car starts, runs, etc as you'd expect it should. Turn key off and then back to start again and it makes ZERO attempt to restart. Reflash SAME bin file back into the pcm and it restarts and runs as it should. Shut it off, and we'll I bet you can imagine what happens.

Scanning with scan94/95 reveals noteworthy items...
WHILE RUNNING:
inj pulse - ~3.85ms
IAC steps - 16
command idle - 800rpm
actual idle - 800rpm (+/-5)

AFTER SHUTDOWN AND RESTART
inj pulse - ~48.5ms
IAC steps - ~140
inj fault this run* set 1
No DTC set (silent or CEL)

ANY HELP WOULD BE GREATLY APPRECIATED!

in eehack, is it flashing both e-side and t-side?

ensure both e-side and t-side are checked, uncheck 'skip unused regions' and 'insert patches' just to see if that helps

... although it shouldn't

Steveo, I was hoping you'd reply. both e-side and t-side are loading. I just reaccomplished a load with skip unused regions and insert patches unchecked. No change. After it successfully starts and I shut it off, I get the "inj flt this run*" error message in scan94/95. Do you know what triggers this? Its after every cycle, then it wont restart. I spoke to GaryDoug and he said he is just the program writer...
